IDENTIFICATION
REQUIREMENTS for a Photo I.D. in Florida
Either TWO documents
(one has to be with a photo); or THREE documents (with no photo), one of which
must be listed below as PRIMARY.
See "Birth Certificates" and "Social Security Cards"
below.
PRIMARY:
Birth Certificate (any state), US
or Foreign Passport, I-94* with Photo, Resident Alien Card, INS Employment Auth
Card (I-688A, I-688B, I-766), US Cert of Naturalization, US Cert of Citizenship,
Drivers License (any state), Florida ID Card, Instruction Permit (any state), Cert
letter of ID for Ward of the Court, Affidavit of ID with Photo (within 15 days
of issue) from Florida Dept of Corrections or county detention facility or probation
office in Florida, Tribal Cert of Indian Blood, Tribal or Bureau of Indian Affairs
Affidavit of Birth, US Military DD-214, US Military ID Card (active duty,
reserve or retired).
SECONDARY:
US Military Dependent ID Card,
US Armed Forces Drivers License, Social Security Card, Motor Vehicle Record or
Clearance Letter (within 30 days of issue), Legal Guardian Affidavit, Selective
Service Card, W-2 Form, Veterans Administration Card, Concealed Weapons Permit,
Medical Insurance ID Card (including AHCCCS Card), Professional License, Bank
Card, Credit Card, Employee ID Badge (with Photo), School ID (with Photo),
Marriage Cert issued in any state, Documents from Court of Record (Divorce
Decree, Adoption Decree, Name Change Decree, Bankruptcy Decree, Emancipation
Decree).
* I-94
form must have photo unless presented with a Florida Refugee Medical Assistance Card
issued by FL DES Refugee Medical Assistance Program. Classifications C-1, C-2,
C-3, D-1, D-2, WT and WB are not allowed a driver license or identification
card.

Social Security Office located at 3733 University Blvd.,
800-234-5772. They are open from 9am to 4pm. You will have to bring the required I.D.
documents listed above to get a replacement card.
