Strategies for Transitioning Ex-offenders Program
A Guide to help you prepare for your release in Duval County, Florida

Many ex-offenders no longer have a family support system when they are released from prison or jail. This STEP Guide is intended to lower the recidivism rate by helping those in need with a Release Program that will help with their transition back into society.

By getting started BEFORE your release and exploring Social Service Programs ahead of time, it will help you to sustain reentry with support from Community-based Programs. These programs are designed to connect individuals who have left the supervision of the justice system with a network of social service agencies and private community-based organizations that provide ongoing support and foster mentoring relationships. Helpful STEP pages give information on where to get started with transportation from jail, emergency shelters, free hot meals, clothing, toiletries, how to get necessary I.D., and where to get day jobs.

Since there is criteria for each Service, it is advisable that you call/write in advance to find out what you need to do, what you need to bring with you, and what the rules are. You can write down what you have found helpful on the “For Your Notes” section, and use the "Calendars" to schedule appointments. Many agencies listed have waiting lists, so don’t expect to get what you need right away. Call in advance, and ask for other referrals. Don’t be discouraged if some of the items have changed. Just keep on trying and asking, and you WILL get the information and help you need.

STEP ONE: Several days before your release, request that a PHOTO "Affidavit of Release" (PAR) to be ready for you when you leave. This will be necessary for shelter, a replacement I.D., and work. 
 

Transportation 

STEP TWO: Get transportation right from jail. An organization by the name of [name and phone number] will pick you up and drive you to an emergency shelter where you can register to stay for the next three nights. Over the next two days you will need to keep the [transporters] busy. Make arrangements to go to:

  • the Social Security Office to get a replacement Social Security Card (bring your PAR).

  • [name of organization] to help get a copy of your Birth Certificate.

  • [name of organization] for two sets of work clothing and boots.

  • a Day Labor office to apply for work to get you started.

It is advisable that you don't have your "friends" pick you up from jail. Start off fresh by not associating with the "old gang." Find new, good friends, who will support you in your efforts to get off the ground on the right foot. People from your church or synagogue would be a good start. Emergency Shelters 

STEP THREE: Get into an Emergency Shelter where you will get clothing, a shower, a hot meal and a warm bed. Make sure you get as much work clothing as allowed at each shelter you go to. Bring your photo Affidavit of Release (PAR). A shelter we highly recommend is:

Provides temporary lodging for three days (once in each 30 day period). Shower, clothing and meals are also part of the free service. Clients are expected to attend short religious services during the three day stay. Trinity Rescue Mission has many programs to help you get started. If you decide NOT to join a program, you will have to leave in three days.

Provides a daily program with supportive services including clothing, shoes, toiletry items, picture identification cards and an information and referral service. Also provides transitional housing (males only) and vocational (culinary) training. An after-school prevention program for at-risk youth ages 6 through 17, is operated in partnership with the city at the Bridge The Gap Community Center.

Shelter and medical care to homeless and uninsured poverty stricken persons living at or below 200% of the Federal Poverty Guidelines. Services are provided on-site at the Sulzbacher Center on a first come first served basis. Services include, but are not limited to the following: primary health, dental, and vision care, pediatric specialty care, asthma clinics, diabetes screening, TB testing, psychiatric clinics, HIV/AIDS testing and case management, minor surgical procedures, and pharmaceutical supplies. Patients are screened and triaged to other specialty care programs as needed and as services are available. Full service shelter (men, women, children) including medical, dental, vision and behavioral health care.
